These homes are located in or around Montgomery County, ensuring that a person will never have to travel too far from home. They will still be allowed to exercise their independence to the greatest degree possible, remaining in a home setting as opposed to a facility.
Because this program is through the Department of Social Services, a complete continuum of care is the goal. Placements are monitored by a social worker, who is there to help locate other available services needed as well as ensure that the placement is meeting the needs of all parties.
